PATHWAYS HEALTH CONNECT

Pathways Health Connect is a free program that helps you connect to the services and support you need to be healthy. Maintaining good health can be difficult, especially when living within limited means and facing additional challenges.

The Pathways care coordination program connects adults, 18 and over, and their families, to a Community Health Worker (CHW). A CHW will give you personal support to help you evaluate what you need. They can advocate for your health needs with insurance companies, doctors, and other providers. They can also help with broader needs that affect your health, like housing, food, transportation, and much more.
